405 Surgical Observations in the Aged, 60 years & over




Abstract
An analysis was made of the records of 405 patients more than 60 years of age, who had operation performed in the surgical dept at the Yonsei University Medical Center in the period
between from May 1963 through April 1965 and from Oct. 1971 to Sep. 1973. The results were as follows:
1. Hospitalitalization and operability rates had to be increased according to increasing number of Korean aged, 60 years and over.
2. According to disease analysis, upper G-I tract disease were more than 55%, of which stomach cancer are estimated nearly 19.3% and are also, the most common one 45% of total patient number were encountered to cancer-group, which is disclosed that cancer is favored to develop -in the aged than in younger. In the non-tumor group. gallbladder and biliary tract diseases were the most common one which are occurred in nearly 13% of total group.
3. There were 239 men and 166 women in the group-a male to female ratio of I :0.7 although the male to female ratio in Korea for total population over the age 60 years is 1: 1. 3.
4. The most common postoperative complication has been mainly pulmonary and affected 103 patients (25.4% to total number). Those is revealed equel ratio to other reports. Cardiovascular complications had less incidence than those of other reports.
5. Postoperative mortality rate was encountered about 5% in this group which is a marked fall than that of other reports due to probably following causes:
1) Selective or planned operations were more than emergency operations. 2) Operability (70%) was less than that of other reports.
3) The age range in this group was more narrow than that of other¢¥ reports.
4) One of the main postoperative causes of death, diseases or complications of cardiovascular origin were less incidence in this group than those of other reports. The most common cause of death was encountered to almost pulmonary complications, about 40%.
6. Thirty-two wounds became infected an over-all wound infection rate of about 8%. Those showed that there is no greater tendency for wound infection in the elderly just because they happen to be old, and, provided that careful techniques are used, a wound healing rate, similar to that for younger patients, could be achieved. It indicated that age, by itself, does not cause increasing of wound infection or delayed wound healing.
